DE435-AA ethernet card is "Unknown device"

I would like some help getting the ethernet controller card up on my DEC
AS1000.  
The card is a DEC DE435-AA "EtherWORKS Turbo PCI".  Neither my original
2.0.36 nor my new 2.2.13 kernel & de4x5.o module can talk to it.  I hope the
following output will help:
